Inpatient Behavioral Health Clinician- LCSW

As a member of an interdisciplinary treatment team, the Inpatient Behavioral Health Clinician LCSW will provide assessment, intervention and aftercare planning for assigned patients on an inpatient behavioral health unit. Assures efficient patient utilization of continuum resources through close communication and collaboration with the patient, primary treatment team, insurers, and community and family support systems. In collaboration with the treatment team, plans appropriate individual and group treatment plans to meet the ongoing needs of assigned patients.

Job Description:

Provides Assessment Completes a psychosocial assessment within department guidelines for timeliness and content. Aims to understand and identify presenting problems and aftercare needs through input from the patient, family, outpatient providers and collaterals (as the patient allows).

Identifies the patient’s strengths and challenges as they pertain to managing symptoms and adherence to an aftercare plan. Identifies when there is a need for a family meeting and/or a network meeting. Provides Casework, Intervention and Aftercare Planning

Attends and participates in treatment team meetings, contributing to the clinical understanding of a patient’s presentation. Shares recommendations for aftercare, progress towards arranging aftercare and identifies obstacles to discharge. Contributes to the Master Treatment Plan.

Organizes and leads family meetings and network meetings as needed.

Provides timely and ongoing contact with family members and collaterals (as the patient allows) throughout hospital stay.

Facilitates and/or provides direct clinical therapeutic interventions to prevent or mitigate relapse and/or readmission.

Works collaboratively with the patient and the treatment team to develop an aftercare plan and to determine a disposition plan. Makes aftercare referrals, schedules aftercare appointments and coordinates services accordingly and per department guidelines.

Maintains collaborative working relationships with network providers and payors to optimize the provision of high-quality patient care.

Solidly understands and follows the expectations and guidelines of regulatory agencies such as DMH, DPH, Joint Commission, payors, etc.

Provides timely documentation (in the electronic medical record) which demonstrates ongoing assessment, aftercare planning and obstacles to discharge.

Provides Education/Advocacy

Provides information/education to patients and their families regarding illness, recovery, support groups, resources and treatment options.

Advocates for patients’ needs.

Identifies and intervenes on behalf of patients whose human rights, safety and/or dignity as a person are in jeopardy as a result of oversight, neglect and/or abuse.

Education, Experience, & Licensing Requirements:

• Master’s degree or doctorate in social work required
• Current valid Massachusetts LCSW license
• At least one year of clinical experience in a behavioral health setting preferred.

Skills/Qualifications:

• Familiarity with use of electronic medical record such as EPIC
• Individual case management and knowledge of managed care
• De-escalation/restraint management techniques
• Motivational Interviewing and short-term treatment techniques
• Brief family interventions as needed
• Strong ability to assess and evaluate patient needs and develop treatment plans to address the identified needs.
• Good communication, listening, and interpersonal skills
• Strong organizational skills
• Ability to be flexible in a fast-paced work environment

Pay Range:

$72,800.00 USD - $97,989.00 USD

The pay range listed for this position is the annual base salary range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law.
