Residents enjoy excellent schools, a reasonable cost of living, and a strong sense of community, making it an ideal home base for healthcare professionals.As a home health Physical Therapist, you will provide one-on-one, skilled therapy services to patients in their homes throughout New Albany and surrounding Southern Indiana communities. You will work with adult and primarily geriatric patients recovering from orthopedic surgeries, neurological events, cardiopulmonary conditions, and chronic illness, focusing on maximizing safety, independence, and quality of life within the home environment. Typical responsibilities include comprehensive evaluations, developing and implementing individualized treatment plans, performing gait and balance training, strength and endurance programs, transfer training, and home safety assessments. You will educate patients and caregivers on home exercise programs, equipment needs, fall prevention, and strategies to remain safely at home.The role is field-based, with a caseload that is typically managed over a Monday through Friday schedule, with the possibility of occasional weekend or on-call rotation depending on agency needs. Daily patient volume is structured to support thorough, high-quality visits and appropriate documentation time. You will collaborate closely with an interdisciplinary team that may include nursing, occupational therapy, speech therapy, social work, and other healthcare professionals, ensuring coordinated care and consistent communication. Patient acuity and visit types can range from post-acute and post-surgical follow-up to long-term management of chronic conditions, offering a clinically diverse and rewarding practice.This position provides a strong professional development environment for therapists interested in honing their skills in community-based care, functional mobility training, and chronic disease management.
