Experience in case management is necessary, along with familiarity with CMS guidelines and benefits eligibility. The candidate will participate in the development of treatment plans and discharge planning. Ability to arrange durable medical equipment and transportation services is required. Competence in managing disease management and insurance company processes is expected. The position involves managing patient care across various settings and potentially floating between facilities as necessary. Knowledge of coding and billing, including ICD 10 and CPT, is preferred. The candidate must ensure compliance with HIPAA regulations. This role may require working weekends and holidays. References are required covering experience within the last three years.
