Acute Care Case Manager

This is a 13-week contract position.

Day shift, 5x8-Hour (08:00 - 16:30) with weekend coverage required.

The Acute Care Case Manager will provide care coordination and management for adult and geriatric patients within an acute care facility. This role focuses on implementing admission criteria, conducting needs assessments, and planning for patient discharges while adhering to clinical standards.

Responsibilities:

• Utilize InterQual criteria for care coordination and admission reviews.
• Conduct assessments for patient care needs and coordinate discharge planning.
• Engage with interdisciplinary teams to ensure comprehensive patient care.
• Manage care plans and prior authorizations as needed.

Required Experience / Certifications / Licensure:

• Minimum of 2 years of recent acute care hospital experience in case management.
• Current California state license required; pending licenses accepted.
• Knowledge of Epic and prior authorization processes.
• Experience working with HMOs and IPAs is preferred.
