Community Health Navigator

Description

A Community Health Navigator (CHN) focuses on promoting community-based health education and prevention in underserved communities. The Community Health Navigator aims to improve health equity for Medi-Cal members through health education and resource referrals by providing outreach with a focus on preventive health services to prevent disease, disability and other health conditions or their progression; to prolong life; and promote physical and mental health and well-being. CHN's increase awareness of services, promote preventative care and reduce emergency visits.

Qualifications:

1. High School diploma or GED required.
2. Proficient in Microsoft suite (Word, Excel, PowerPoint).
3. Strong computer skills/data entry.
4. Knowledge of community resources.
5. Bilingual in Spanish required.
6. A valid California driver's license is required of staff who have regular driving duties and the driving record must meet the requirements of their position and be approved by the agency's insurance company.

Essential Job Functions:

1. Operate an agency or personal vehicle for work-related activities which may include, client visits, meetings, trainings, or other agency-related appointments
2. Ensure compliance with driving requirements, including maintaining a valid driver's license, current vehicle registration, and proof of insurance as required by agency policy.
3. Ability to communicate ideas in oral and written form.
4. Ability to instruct in group and individual settings.
5. Physical, mental, and emotional health adequate to perform duties.
6. Ability to work effectively with cultural/ethnic diversity.
7. Protect and honor individual integrity through confidentiality.
8. Ability wo work in a harmonious manner with subordinates, supervisors, and others.
9. Adhere to all applicable legal and ethical standards.

Responsibilities:

1. Utilize prevention strategies to promote stigma and discrimination reduction, suicide prevention, mental health awareness and well-being.
2. Provide access, advocacy, resources and linkage to clients and their families.
3. Interact productively within a multi-disciplinary team and other community organizations.
4. Maintain record and tracking systems that meet standards of the agency, The Joint Commission and contract.
5. Must implement program goals and objectives to meet and maintain contractual requirements and productivity.
6. Responsible for preventing and controlling infection.
7. Responsible for maintaining a culture of quality and safety.
8. Effectively use de-escalation skills and techniques for crisis prevention.
9. Properly administer the C-SSRS to help prevent suicide

Other:

1. Attend all required meetings and trainings.
2. Report any suspected child or dependent adult/elder abuse or neglect immediately to direct supervisor or utilize the chain of command if supervisor is unavailable.
3. Report any client imminent danger to self or to others or gravely disabled immediately to direct supervisor or utilize the chain of command if supervisor is unavailable.
4. Other duties as assigned.
