Reporting:This position reports directly to the Physical Therapist and collaborates with the rehabilitation team and interdisciplinary staff.

Role Overview

• Implement physical therapy treatment plans under PT supervision
• Document therapy sessions and resident progress accurately andtimely
• Assistresidents with therapeutic exercises, mobility, and functional activities
• Collaborate with Physical Therapists and the interdisciplinary team
• Participate in care conferences and therapy-related activities, as applicable

WhatWe'reLooking For

• Active California Physical Therapist Assistant license in good standing
• Associate degree from an accredited Physical Therapist Assistant program
• New graduates are welcome to apply; experience in skilled nursing or rehabilitation settings preferred
• Knowledge of PDPM and basic documentation standards
• Strong communication, teamwork, and interpersonal skills
• Compassionate, resident-centered approach to care
• Physical ability to safelyassistresidents with mobility and therapeutic activities
