Foster Care Case Navigator

Heartshare seeks a Floating Case Planner to assist division case planners with day-to-day social services tasks. You will help connect families to services, support reunification efforts, and may temporarily fill a vacant case planner position to cover visits and safety contacts. You will prepare service plans with families, monitor safety and progress, and participate in court hearings and Family Team Conferences to advance permanency.
