Responsibilities
- Conduct comprehensive speech, language, cognitive-communication, and swallowing assessments for adult residents.
- Develop, implement, and monitor individualized therapy plans and goals; document progress and outcomes.
- Provide direct therapy, bedside interventions, and group sessions as appropriate; adjust plans based on patient response.
- Collaborate with an interdisciplinary team (nursing, rehabilitation, physicians) to support discharge planning and functional goals.
- Educate patients and families on communication strategies, compensatory techniques, and safe swallowing practices; document counseling notes.
- Ensure compliance with facility policies, HIPAA, and payer requirements; participate in care conferences and quality initiatives.

Requirements
- Current California license to practice as a Speech-Language Pathologist (S-LP)
- Master’s degree in Speech-Language Pathology
- Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred; or eligibility for CCC
- Relevant experience: minimum 1 year of recent clinical experience in acute care or skilled nursing facility settings
- Ability to work in a skilled nursing environment and meet a minimum 35 hours per week
- Willingness to travel for assignments and complete onboarding credentialing
