Care Specialist

Are you an empathetic and compassionate caregiver who takes pride in helping people lead more independent lives? Our busy home health agency is seeking an experienced home health aide to assist our case managers in implementing patient-specific care plans. You’ll give our patients companionship and help with basic medical care, light housekeeping duties, personal care, and hygiene tasks so that they are empowered to live happily at home. You must have 1-3 years of experience as an HHA or caregiver and possess excellent communication skills. If you enjoy a position that is varied and rewarding every day, we want to hear from you! Responsibilities: • Transport patients as needed to doctor’s offices or other health care services, grocery shopping, or for other errands • Implement ambulation methods, and assist patients in completing physical therapy and exercises as indicated in the care plan • Complete basic medical tasks such as taking vital signs, measuring glucose and ketone levels, replacing bandages and dressings, and ensuring the proper use of medical devices • Supervise medications and assist patient in organizing, refilling, and taking medications, as well as meal planning and preparation • Stay on top of light housekeeping tasks including but not limited to laundry, bed making, vacuuming, etc. Administer Medications: Dispense medications as directed by the Registered Nurse (RN), maintaining an annual error rate of no more than one mistakeResident Response: Attend to resident call lights within 10 minutes to ensure responsive and attentive careLight Housekeeping: Perform regular light housekeeping in resident rooms at least once weekly to promote a clean and comfortable living spaceService Charting: Accurately document all services provided without missing tasks, ensuring compliance with company standardsAssistance with ADLs: Support residents in Activities of Daily Living (ADLs) according to each individual’s Client Service Plan, with a zero-tolerance policy for missed tasks Qualifications: • Physical requirements include being able to lift up to 100 lbs, may be required to stoop, bend, reach, and transfer patients up to 250 lbs as needed • Possess a valid state driver’s license, current auto insurance, and reliable transportation • Compassionate, kind, and empathetic personality is essential, with superb organization and communications skills • A high school diploma or GED equivalent is required - some college is an asset • First aid training and current CPR certification is needed and you must be able to pass a drug test, full employment background check, and physical Some skills expected- Ability to lift 35 lbsAssist with resident ADLs as directed by Client service planAdministration of medication as directed by an RNAnswer client call lightsHousekeeping in residents' rooms as scheduledCharting of all services performedAttributes: Extreme Ownership: Demonstrates accountability, and ownership of outcomes, and proactively removes obstaclesResourcefulness: Finds effective solutions even with limited resources and independently seeks knowledgePassion for Purpose (Big Why): Possesses a strong, genuine dedication to serving and caring for vulnerable adultsCommitment to Growth: Actively pursues self-improvement and team betterment each day.Humility: Embraces a solutions-focused attitude, welcoming feedback and putting egos aside for the best outcomeReliability: Shows up on time, prepared, and ready to deliver dependable supportGratitude: Maintains a positive outlook, seeking and appreciating the good in all situations Compensation: $16 - $18 hourly
