Your key responsibilities will include: Conducting skilled nursing visits and coordinating client care Serving as the primary contact for all disciplines involved in patient care Monitoring the frequency and quality of patient visits Consulting with physicians regarding patient care details Accurately collecting OASIS data and ensuring medication profiles are current Reporting lab values to physicians in a timely manner Participating in weekly case conferences and monthly case manager meetings Qualifications To be considered for this role, Registered Nurses must: Hold a currently valid RN license in the state Have at least one year of clinical experience (preferred) Demonstrate knowledge and skills in current nursing practices Possess a valid state driver’s license and automobile liability insurance Sign-On Bonus We offer a $5,000 Sign-On Bonus!
