Serve as a liaison among the patient, family, nurse, physicians, multidisciplinary teams, and the patient’s healthcare benefactor to optimize outcomes. Act as a consultant to the healthcare team on specific patient items. Coordinate a continuum of care for a defined population of patients from pre-admission through post-discharge. Ensure patients and families progress efficiently through the course of hospitalization and discharge planning. Requirements Active Registered Nurse license in California. Experience in case management or care coordination (preferred). Strong communication and collaboration skills. Ability to work in a hospital/acute care setting. Knowledge of discharge planning and care transitions.
