Service CoordinatorFull Time | Day ShiftPay: $41,600-$45,760 annually based on experienceServing multiple Genacross communities: Luther Ridge, Luther Hills, Luther Grove, Covenant Harbor I, Covenant Harbor II, and Oak HarborAt Genacross Lutheran Services, we've been serving individuals, families, and communities with compassion for more than 160 years. As a faith-based nonprofit organization, we are dedicated to putting our mission into action every day - serving the needs of our community with exceptional care, innovation, and support.What You'll Do: As a Service Coordinator, you will serve as a trusted resource for residents by helping them identify and access community services that support healthy, independent living.Assess resident needs and coordinate appropriate supportive services and resourcesMaintain accurate resident records, assessments, progress notes, and documentationConduct annual resident apartment visits and update resident files as neededMonitor resident concerns and communicate important issues to leadership promptlyAssist residents in navigating available community programs and benefitsReport suspected abuse, neglect, or exploitation in accordance with regulatory requirementsComplete monthly reports and maintain compliance with program requirementsSupport partnerships with local organizations, faith communities, and service providersAssist with PASSPORT Certification documentation and reportingServe as a Transition Coordinator for assigned Home Choice Program referralsParticipate in required training and professional development opportunitiesHelp ensure high-quality, resident-centered service deliveryService Coordinator Requirements:Valid driver's license Bachelor's degree in social work or related field preferredLSW/LISW preferredStrong oral and written communication skillsUnderstands directions, communicates and responds to inquiries promptlyExcellent human relation skillsWhy Join Genacross?At Genacross, you'll be part of a mission-driven team.
