Case ManagerThe Case Manager is a Registered Professional Nurse who demonstrates expanded knowledge and experience in providing care for an identified clinical specialty or case type. Responsibilities include, but not limited to: Accountable for desired clinical and cost-effective outcomes, collaborative practice, the allocation of human and material resources needed for care delivery, discharge planning, and evaluation of length of stay. Utilizes the case management process to perform a leading role in improving the quality and efficacy of care provided. Develops integrated and coordinated approach to patient outcomes, and monitors care delivery and interdisciplinary clinical practice patterns to achieve a balance between quality and cost (desired outcomes). Practices according to the established standards for case management and has attained competencies. Will demonstrate the knowledge and skills necessary in providing case management practices which are appropriate to the age of the patient. Understanding of the unique physical and psychological needs of the pediatric, adolescent, adult, and geriatric patients as demonstrated.
