Your future role at a glance Location: Lockhart, Texas Facility: Seton Lockhart Health Center Department/Speciality: Long Term Care Clinic Schedule: Full time | Monday-Friday 8am-5pm How you'll make an impact in this role Lead patient assessments to proactively identify changes in condition, ensuring timely interventions and a safe environment for healing. Administer medications and treatments with precision, while vigilantly monitoring patient responses to ensure the highest standards of clinical safety. Empower patients and families through personalized education, providing the tools and knowledge they need for a successful recovery and long-term wellness after discharge. Collaborate with the multidisciplinary team to execute therapeutic services and lab tests, acting as a critical link in the delivery of high-quality, holistic care. What minimum qualifications you'll need Licensure / Certification / Registration: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. Licensed Practical/Vocational Nurse credentialed from the Texas Board of Nursing required. Education: High School diploma equivalency or 1 year of applicable cumulative job specific experience required. Note: Required professional licensure/certification can be used in lieu of education or experience, if applicable. What additional requirements you'll need Clinical Qualifications & Experience: Holds an active LVN license with background in Internal or Family Medicine, alongside hands‑on proficiency using the Athena EMR platform. Core Competencies & Capabilities: Demonstrates strong customer service, problem‑solving, and multitasking abilities, with the independence to work autonomously and flexibility to travel as needed.
