Physical Therapist Assistant

Confluent Health System Solutions seeks a Physical Therapist Assistant to support high-quality, evidence-based rehabilitation care. In this collaborative, mission-driven role, you’ll implement treatment plans under the supervision of a Physical Therapist, deliver therapeutic exercises and functional training, and educate patients and families on home programs. You’ll document outcomes in EMR systems, contribute to care coordination across clinical teams, and participate in continuous learning and process improvement to enhance patient function and health system performance.
