Your Qualifications:

• Graduate of nationally accredited Physician Assistant (PA) or Nurse Practitioner (NP) Program
• NPs should have Family Certification
• Ability to obtain PA/NP licensure in Wisconsin
• Experience as a practicing provider, f amily medicine experience highly preferred
