Hospice RN Case Manager: Lead Compassionate Care

Gentiva Heartland Hospice in Oklahoma seeks a dedicated Hospice RN / RN Case Manager to deliver compassionate, patient-centered care in patients' homes or other settings. You will guide families, document care, and collaborate with physicians, LPNs, CNAs, social workers and chaplains to implement personalized plans of care. The role emphasizes education for patients and caregivers, timely communications, and adherence to infection control and regulatory standards as part of a patient-focused #J-18808-Ljbffr Gentiva
