In-Home Nurse Practitioner or Physician Assistant

Signify Health is hiring an In-Home Nurse Practitioner or Physician Assistant to conduct comprehensive health evaluations for patients in their homes. You will perform physical exams, review medical histories, reconcile medications, and identify care gaps for value-based care programs. Using mobile technology and analytics, you’ll document findings, coordinate with primary care and other providers, and educate patients on disease management. You’ll work independently in the field while collaborating with interdisciplinary teams to improve outcomes for vulnerable, high-risk populations.
