We are currently seeking a motivated, detail-oriented Kaiser Case Manager LVN to join our collaborative interdisciplinary team. In this role, you will serve as the primary liaison for our Kaiser Permanente residents, ensuring seamless care coordination, utilization review, and comprehensive discharge planning. Utilization Review: Complete regular utilization management reviews using standardized criteria (e.g., InterQual) to optimize appropriate lengths of stay and resource allocation. Interdisciplinary Collaboration: Act as the central point of contact between Kaiser health plans, facility physicians, therapy departments, nursing staff, and families. Discharge Planning: Coordinate safe transition plans, arrange home healthcare/durable medical equipment (DME), and prevent unnecessary hospital readmissions. Documentation & Compliance: Maintain precise, timely case management documentation in compliance with federal, state, and managed care contract guidelines. Qualifications & Requirements Licensure: Active, unrestricted California Licensed Vocational Nurse (LVN) license. Experience: Minimum of 1–2 years of nursing experience; prior case management, utilization review, or managed care experience (specifically working with Kaiser systems) is highly preferred. Skills: Strong organizational capacity, exceptional communication skills, computer literacy, and an ability to navigate complex medical files. Knowledge: Familiarity with skilled nursing facility (SNF) regulations, Medicare guidelines, and insurance authorization processes. We are an equal opportunity employer.
