Minimum Qualifications

• Associate degree in Physical Therapy from an accredited educational program.
• Active New York State PTA license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Prior skilled nursing facility (SNF) experience required. Candidates must be able to hit the ground running with SNF caseloads and varied diagnoses.
• Ability to work within a multidisciplinary team to plan safe discharges and transitions of care.

Preferred Qualifications

• Experience working in a 100+ bed skilled nursing or long-term care setting is preferred.
• Experience providing therapy for varied diagnoses and supporting discharge planning is preferred.
• Experience collaborating with OT and SLP within a rehabilitation team is preferred.
