• Conduct initial and comprehensive assessments of residents' medical needs, including the development of individualized care plans

• Collaborate with multidisciplinary healthcare teams to ensure effective communication and coordination of care

• Monitor residents' conditions and adjust care plans as needed to ensure optimal health outcomes

• Maintain accurate and detailed records of resident assessments, care plans, and treatment progress

• Participate in quality improvement initiatives to ensure compliance with regulatory standards and industry best practices

• Demonstrate a commitment to delivering resident-centered care, prioritizing their physical, emotional, and psychological needs
