Physical Therapist Assistant - Home Health

Role: The Physical Therapist Assistant provides treatment, exercise instruction, and patient education to improve patient well-being, following a designated care plan under supervision. Responsibilities Perform therapy procedures aligned with state laws and trained scope, recommending care plan adjustments to the supervising PT. Instruct and assist patients with exercises, gait, ADL, transfers, safety, and prosthetic training. Monitor and report patient responses and condition changes to the healthcare team, coordinating care as needed. Educate patients, families, caregivers, and team members about therapy activities. Adapt to various home environments, regardless of patient demographics. Qualifications & Skills Current license, registration, or certification in physical therapy assistance within the practicing state. Valid drivers license, vehicle insurance, or access to transportation. Availability for weekend rotations. Preferred: CPR certification (or ability to obtain within 90 days), physical ability for patient mobility tasks.
