Manager

Job Title

To be fully engaged in providing No Harm/Quality care, Customer Experience, and Stewardship through the coordination, implementation, and optimization of appropriate, patient-focused care while ensuring clinical, customer/patient satisfaction, regulatory and financial goals are being met. The manager is responsible for supervision of supervisory and clinical staff, the intake department, and clinical liaisons, while supporting the IDN in a collaborative relationship with our hospital leaders, medical group and community partners.

Primary Accountabilities

1. Lead daily clinical operations to ensure quality patient care, appropriate utilization, regulatory requirements, and customer/patient satisfaction.
2. Demonstrate and promote strategic thinking and drive problem-solving by empowering and supporting clinicians to assume responsibility and accountability within their scope of practice.
3. Maintain and disseminate best practice clinical information obtained from across Health First and from external resources.
4. Provide performance tools and a system of accountability by proactively monitoring leading indicators for patient safety, comparing performance outcomes against top decile benchmarks, and ensuring action planning to continuously improve patient outcomes and customer experience.
5. Oversee and manage the development of appropriate policies and procedures to ensure compliance with the necessary regulatory agencies including AHCA, The Joint Commission, etc. Monitor and ensure compliances with resource allocation.
6. Direct quality and efficient care coordination to ensure an optimal member/patient experience and maximize customer satisfaction.
7. Lead business development initiatives for strategies to build referral volume by managing hospital and non-hospital liaison team to achieve financial benchmarks.
8. Manage the clinical intake referral team by ensuring the safety and appropriateness of patient acceptance and facilitate timely initiation of care with proper disciplinary team members.
9. Serves as an IDN partner by mediating and assisting with complex problem solving, conflict resolution, and prioritization utilizing continuous improvement principles.

Work Experience

Minimum Qualifications

• Education: BSN
• Work Experience: Three (3) years Healthcare leadership experience.
• Licensure:

1. Current RN Licensure in the State of Florida or endorsement.
2. Valid Drivers License and clean Motor Vehicle Record and maintained.

• Certification: American Heart Association (AHA) Basic Life Support (BLS) Healthcare Provider Completion Card prior to start date and maintained.
• Background Check: Must satisfy Florida of Agency for Health Care Administration (AHCA) Level 2 background screening.
• Skills/Knowledge/Abilities:

1. Oasis documentation.
2. CMS Conditions of Participation.
3. ACHA Home Care Standards.
4. TJC Standards.

Preferred Qualifications

• Work Experience: Homecare leadership experience.

Physical Requirements

• Majority of time involves sitting or standing; occasional walking, bending, and stooping.
• Long periods of computer time or at workstation.
• Lightwork that may include lifting or moving objects up to 20 pounds with or without assistance.
• May be exposed to inside environments with varied temperatures, air quality, lighting and/or low to moderate noise.
• Communicating with others to exchange information.
• Visual Acuity and hand-eye coordination to perform tasks.
• Workspace may vary from open to confined.
• May require travel to various facilities within beyond county perimeter may require use to personal vehicle.
