Key Responsibilities

• Evaluate and treat patients in a skilled nursing facility setting to restore function and improve mobility
• Develop and implement individualized physical therapy treatment plans based on patient needs and goals
• Document patient progress, treatment outcomes, and clinical notes in accordance with facility and regulatory standards
• Collaborate with interdisciplinary care teams to coordinate comprehensive patient care
• Educate patients and caregivers on therapeutic exercises and home programs to support continued recovery

Required Qualifications

• Active Physical Therapist (PT) license in the state of Oregon or ability to obtain prior to start
• Graduate of an accredited Physical Therapy program
• Current BLS/CPR certification
• Prior experience working in a skilled nursing or long-term care setting

Preferred Experience / Skills

• Experience with MDS assessments and PDPM documentation
• Familiarity with electronic medical records (EMR) systems used in SNF environments
