What You’ll Do Provide skilled nursing visits in homes and community settings. Perform assessments, wound care, and medication management . Educate patients and families with clarity and compassion. Collaborate with RN Case Managers and the care team. Float to nearby territories to support patient needs. Maintain accurate, timely documentation .
