Requirements: Current state license as a Registered Nurse; CHPN certification; Proof of current CPR certification; Current driver's license; Must have reliable transportation and insurance; 1-2 years experience as a Registered Nurse in a clinical care setting; 2+ years Home health experience

Responsibilities:

• Assess home health and hospice patients to identify physical, psychosocial, and environmental needs.
• Develop and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
• Educate patients and families regarding care, medications, and safety.
• Supervise and provide clinical directions to Home Health Aides/Staff.
• Monitor and manage pain, educate on pain management techniques.
• Arrange referrals to appropriate agencies as needed.
• Perform on-call responsibilities and provide services to patients and families.
• Serve as a liaison between patients, families, and healthcare team members.
• Attend interdisciplinary team meetings and other relevant gatherings.
