Shift & Schedule: Day Shift | 8:00 AM – 4:30 PM | Monday – Friday | 5×8 Hours

Pay Rate

Local: $55 - $60/hr

Position Overview

We are seeking an experienced Medical Social Worker to provide psychosocial support, discharge planning, and care coordination for patients in an inpatient acute-care setting.

Required Qualifications

• Master’s Degree in Social Work (MSW) required.

• Minimum 2 years of inpatient acute-care experience.

• Epic EMR experience required.

• Strong discharge planning and interdisciplinary collaboration skills.

• Ability to participate in one weekend day per month rotation.

Responsibilities

• Complete comprehensive psychosocial assessments for hospitalized patients.

• Develop and coordinate safe discharge plans.

• Provide crisis intervention, counseling, and emotional support.

• Connect patients and families with community resources and post-acute services.

• Collaborate with physicians, nurses, and case management teams.

• Document patient care accurately in Epic EMR.

• Support patient care coordination and continuity throughout the hospital stay.
