We serve individuals with Intellectual/Developmental Disabilities (I/DD) , Serious Persistent Mental Illness (SPMI) , and Serious Emotional Disturbance (SED) —and we’re looking for dedicated professionals to join our team. Position: ACT Case Manager As a member of our Assertive Community Treatment (ACT) team , you’ll help individuals with severe and persistent mental illness manage symptoms, integrate into the community, and achieve the highest possible quality of life.
