This role focuses on improving mobility, safety, and independence while supporting patients in their natural living environment. Opportunities are available for travel and local contracts . - Perform in-home evaluations and develop individualized plans of care
- Provide skilled physical therapy treatments focused on mobility and safety
- Educate patients and caregivers on exercises and fall prevention
- Coordinate care with physicians and interdisciplinary teams
- Complete accurate and timely documentation
- Doctorate or Master's Degree in Physical Therapy
- Active (or pending) state PT license
- Home Health experience preferred, not required
- Strong time management and communication skills
