Job Details: Schedule: 8 AM-4:30 PM Location: Russellville, AR Job Type: Contract Contract Length: 13 weeks (possibilities of extension) Pay Range: $52/hr- $56/hr *Pay offered is based on experience, expertise, credentialing, and education.

Key Responsibilities

• Perform comprehensive patient evaluations and develop individualized plans of care
• Provide skilled physical therapy services in patients' homes
• Conduct Start of Care (SOC), Resumption of Care (ROC), recertifications, and discharge visits
• Monitor patient progress and adjust treatment plans as needed
• Educate patients and caregivers on exercises, safety, and ongoing care
• Maintain timely and accurate clinical documentation in compliance with regulations
• Collaborate with physicians, nurses, and interdisciplinary team members
• Ensure compliance with Medicare guidelines and home health regulations

Preferred Qualifications

• Experience with OASIS documentation Familiarity with home health EMR systems

Requirements:

• Education: Degree in Physical Therapy (PT)
• License(s): Active Physical Therapist license
• Certification(s): BLS
• Years of experience: 1 year
• Skills/Specialties/Technology: Home Health
