As an Advanced Practice Nurse at Katy Trail Community Health, you will start each day by engaging with patients, assessing their needs, and providing comprehensive care. Your daily responsibilities include conducting thorough physical examinations, diagnosing conditions, and developing individualized treatment plans. You will also collaborate with a multidisciplinary team to ensure optimal patient outcomes. Expect to spend your Monday to Friday schedule from 8:00 AM to 5:00 PM actively participating in patient care, documenting medical histories, and providing education to patients and their families. You'll have opportunities to participate in team meetings where you'll discuss case management and innovative care strategies.
