Home Health Physical TherapistPride Health is hiring a Home Health Physical Therapist to support our client's medical facility, located in Puyallup, WA 98373. This is a 3+ month contract assignment (possibility of extension or conversion) and a great way to start working with a top-tier healthcare organization!Job title: Home Health Physical TherapistLocation: Puyallup, WA 98373Duration: ContractPay Range: $50 - $56 on w2/Pay rate is based on experience and educational qualifications.Shift: Mon - Fri 8am - 4:30pm.Travel: Patient homes - Pierce County, WAMileage Reimbursement: Yes (between patient visits).Specific Duties:Evaluate patients' functional status and physical therapy needs in compliance with CMS guidelines.Develop and implement individualized treatment plans in collaboration with physicians.Deliver evidence-based therapy to relieve pain, restore function, and improve mobility.Monitor and document patient progress, reporting changes to physicians and nurse supervisors.Educate patients, families, and caregivers on treatment plans, progress, and functional limitations.Complete timely and accurate documentation, including evaluations, progress notes, and discharge summaries.Travel to patient homes within the assigned service area and nearby locations as needed.Participate in care coordination, team meetings, and case conferences.Supervise Physical Therapist Assistants (PTAs) and Home Health Aides as applicable.Maintain compliance with agency policies, state regulations, and professional standards of practice.Participate in continuing education to maintain licensure and clinical competency.Qualifications:Active Physical Therapist license in Washington State (or compact license).Current CPR certification.Valid driver's license, auto insurance, and reliable transportation.Experience in home health or related clinical settings preferred.Pride-Health offers eligible employee's comprehensive healthcare coverage (medical, dental, and vision plans), supplemental coverage (accident insurance, critical illness insurance and hospital indemnity), 401(k)-retirement savings, life & disability insurance, an employee assistance program, legal support, auto, home insurance, pet insurance and employee discounts with preferred vendors.
