What You’ll Do:

• Provide comprehensive outpatient primary care to adults and families
• Diagnose and treat acute and chronic conditions
• Conduct wellness exams and routine follow-ups
• Develop long-term care plans in coordination with specialists
• Document patient encounters using EPIC EMR
• Collaborate with multidisciplinary teams to ensure quality outcomes
• Participate in shared on-call duties
• Engage in quality improvement initiatives
• Build strong patient-community relationships
• Support continuity of care and patient education
• Adhere to organizational standards and compliance

What You Need:

• State medical license and Board Certification in Family or Internal Medicine (or eligibility)
• 0+ years of clinical experience preferred
• Strong communication and collaboration skills
• Ability to work independently and as part of a team
