What Makes This Role Stand Out: Consistent Weekend Hours: Fit work around your life with weekend-only scheduling Excellent Pay: $200 per Start of Care visit + mileage Focused Role: Specialize in new patient admissions and care plan initiation Community-Based: Get out of the office and into the homes of clients across the metro Professional Independence: Use your clinical judgment to help patients begin their care journey Primary Responsibilities: Perform weekend Start of Care (SOC) admissions with complete OASIS documentation Develop and initiate care plans in collaboration with the interdisciplinary team Evaluate patient needs and coordinate necessary services Educate patients and caregivers on disease management and home care best practices Required Qualifications: Valid Minnesota RN license and CPR certification Proven experience in home health and OASIS documentation Reliable transportation, driver's license, and active insurance Strong communication and clinical assessment skills Job Title: Weekend Admission RN – Home Health Care Expert Company: Home Health Care, Inc.
