Physical Therapist

Schedule: 8-hour shifts

Assignment Details:

• Contract Length: 13 weeks
• Guaranteed Hours: 40 hours per week

Requirements:

• Current State License as a Physical Therapist
• Proof of current CPR certification
• Current driver's license. Must have reliable transportation and insurance
• 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting
• Previous home care experience - Preferred
• Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program

Roles & Responsibilities:

• Perform clinical assessments to identify patient needs and determine appropriateness for physical therapy.
• Document and complete patient care plans, progress notes, and other clinical record documentation on an ongoing basis.
• Plan and provide treatment for patients with neuromuscular, neurological, and orthopedic disorders based on medical referrals and therapy assessments.
• Educate patients and families on therapeutic procedures and safe use of rehabilitative equipment.
• Ensure compliance with infection control and universal precautions in patient care activities.
• Collaborate with the interdisciplinary team and adapt treatments based on ongoing evaluation and patient progress.
