We are seeking a licensed and experienced LPN to join our dedicated team of healthcare professionals. As an LPN at Allaire Health Services, you will play a vital role in delivering high-quality patient care to our residents. Your responsibilities will include:

• Providing direct patient care, including administering medications, performing vital signs, and assisting with daily living activities

• Developing and implementing individualized care plans in collaboration with the interdisciplinary team

• Conducting assessments and reporting changes in patient condition to the nurse manager

• Assisting with treatments, procedures, and medical tests as ordered by the attending physician

• Maintaining accurate and up-to-date patient records

Requirements:

• Current LPN license in the state of Vermont

• 1+ year of experience as an LPN in a long-term care setting

• Strong clinical skills and ability to work effectively in a fast-paced environment

• Excellent communication and interpersonal skills

• Ability to work a variety of shifts, including days, evenings, and weekends
