Profile Summary LOCATION: Must reside in one of the following Illinois Zip Codes; 60607, 60608, 60612, 60622, 60623, 60624, 60639, 60642, 60644, 60647, 60651 This Analyst, Case Management Field position is with Aetna’s Long Term Services & Supports (LTSS) team and is a field-based position in Chicago, Illinois. The role requires traveling 50-75% of the time to meet with members face to face. The position holds a full caseload to manage waiver members and requires in-person home visits. The analyst facilitates appropriate healthcare outcomes for waiver/LTSS members by providing care coordination, support and education for members through Required Qualifications Minimum of two (2) years of case management experience Microsoft Office and electronic health record experience Must possess reliable transportation and be willing and able to travel up to 75% of the time. Mileage is reimbursed by our company expense reimbursement policy Valid Illinois Driver’s license Preferred Qualifications Case management and discharge planning experience Managed Care experience Microsoft Office experience Experience working with HIV and AIDS population Multi-lingual language proficiency Required Education Individual with a bachelor’s degree or a non‑licensed individual with a master’s degree in a human‑services field (including, but not limited to sociology, special education, rehabilitation counseling) Anticipated Weekly Hours: 40 Time Type: Full time Pay Range: $21.10 - $44.99 This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above. Our people fuel our future.
