As a Hospice RN Case Manger, you will manage a caseload of patients receiving hospice care in their homes and care facilities. You will work closely with an interdisciplinary hospice team to ensure comfort, dignity and quality of life.

Responsibilities

• Conduct thorough patient assessments, including vital signs, physiological evaluations, and symptom management for hospice patients.
• Develop and update individualized care plans.
• Collaborate with interdisciplinary team to provide seamless care
• Monitor patient progress through ongoing evaluation of symptoms, medication management, and response to interventions.
• Document all patient interactions accurately within medical records while maintaining strict adherence to HIPAA regulations.
• Educate patients and families on disease progression, symptom control, medication use, and available support resources.
• Participate in on-call rotation as needed.
