Key Responsibilities\n

  • Conduct detailed psychosocial assessments to understand patients' emotional and social needs, utilizing advanced interviewing techniques.

  • Implement therapeutic interventions, including individual, group, and family therapy, to foster adjustment and resilience among patients and their families.

  • Work closely with an interdisciplinary team to develop and implement personalized treatment plans that address the unique challenges of each patient.

  • Coordinate case management for assigned patients, ensuring they receive appropriate resources from the community, such as education on healthcare benefits or support groups.

  • Respond effectively to crisis situations, providing immediate support and intervention in emergencies such as mental health crises or risk of self-harm.

  • Monitor and regularly follow up on patients' progress, adapting treatment plans as necessary to ensure optimal care and support.

Patient Population / Client Focus\n

The primary focus of this role will be on patients receiving hospice care, many of whom are navigating significant life transitions due to serious illnesses. You will be working with a diverse population, including individuals and families from various backgrounds and cultures, requiring sensitivity and a strong understanding of end-of-life issues.

Qualifications\n

  • A Master’s Degree in Social Work from an accredited graduate program is required.

  • A valid California Board of Behavioral Sciences Licensed Clinical Social Worker (LCSW) credential is mandatory.

  • At least 2 years of experience in a behavioral health setting, particularly with a focus on end-of-life care.

Preferred Experience\n

  • Experience working with Veterans and understanding their unique needs is highly desirable.

  • Proficiency in taking psychosocial histories and conducting comprehensive case management evaluations, including assessing patients for potential suicidal ideation.

  • Demonstrated skills in delivering individual, group, and family therapy interventions in a compassionate and effective manner.

  • A solid understanding of community resources and how to navigate them on behalf of clients is crucial.

Compensation and Benefits\n
