Registered Nurse - Home Health

Job Description

Job Summary:

Provides professional nursing care within a home health environment, ensuring adherence to regulatory standards and safety protocols. Coordinates patient care plans and supervises nursing assistants to deliver holistic patient support.

Responsibilities:

• Prescribe, coordinate, and deliver nursing care through the nursing process.
• Conduct comprehensive initial assessments and ongoing evaluations of patients' physical, psychosocial, and environmental needs.
• Develop, implement, and modify individualized care plans, including discharge planning.
• Supervise CNAs and collaborate with other healthcare team members to ensure quality care delivery.
• Document patient care activities accurately according to regulatory guidelines and agency protocols.
• Promote a safe, clean, and confidential environment aligned with industry standards and regulations.

Qualifications:

• Valid Illinois Registered Nurse license and active license status required.
• Home health experience, including OASIS data collection and start of care assessments, strongly preferred.
• Active BLS certification or willingness to obtain prior to start.
• Valid driver's license and reliable vehicle, along with current car insurance.
• Strong clinical skills in chronic disease management, wound care, and patient education.
• COVID and flu vaccinations are required; declinations are not approved.
