Key Responsibilities\n- \n
- Conduct comprehensive evaluations of clients' needs to inform treatment plans tailored to their unique challenges and goals. \n
- Facilitate effective communication among multidisciplinary teams, including supervisors, direct care staff, families, and external professionals, to collectively monitor client progress. \n
- Develop organized schedules that encompass patient appointments, home visits, and relevant observations for comprehensive care tracking. \n
- Perform essential home visits to complete in-depth assessments, including chart reviews and staff interviews, while analyzing collected data to optimize treatment delivery. \n
- Ensure adherence to established program plans, implementing therapeutic interventions with fidelity and creativity. \n
- Participate actively in Interdisciplinary Team Meetings to contribute insights and recommendations for enhancing patient care. \n
- Prepare and submit monthly and quarterly reports analyzing progress toward treatment objectives. \n
- Coordinate training workshops to enhance staff knowledge and competency regarding program implementation and documentation practices. \n
- Conduct follow-up interviews and observations to sustain consistent support for clients' evolving needs. \n
- Maintain organized records to guarantee that all pertinent information is accessible to authorized personnel. \n
- Act as a vital connector between the general population and mental health resources, advocating for patients and guiding them toward their development goals. \n
- Assist in creating and updating training materials to ensure that all therapeutic programs are executed effectively and creatively. \n

Qualifications\n- \n
- A Master's or Doctoral degree in Counseling, Psychology, or a closely related field is mandatory. \n
- Current and unrestricted state license as a Professional Counselor (LPC) in Texas. \n
- A minimum of two years of professional counseling experience, preferably in a clinical environment. \n
- Specialization in areas such as family therapy, trauma-informed care, or substance abuse counseling is highly valued. \n
- Familiarity with legal and ethical guidelines governing mental health practices. \n
