Licensed Practical Nurse - North Streams Urgent Care

Job Summary Location: Methodist Physicians Clinic – North Streams Urgent Care, Address: 4932 S 202nd Ave – Omaha, NE. Work Schedule: Monday through Friday, 10am – 10pm with rotating weekends and holidays 12pm – 6pm. Must be willing to float to other urgent care locations if needed. Licensed Practical Nurse (LPN) working at an Urgent Care Clinic, responsible for evening and weekend shifts. Assists physicians by reviewing records, preparing patients and exam rooms, and handling prescription renewals and inquiries. Responsibilities Medical Home. Documents all lab, procedures, and appointments on tracking forms. Completes tracking forms and follows up on missed appointments and procedures. Assists Health Coaches by working patient lists and scheduling appointments as directed. Provides patient and family education and recommends community resources as needed. Performs telephone triage to determine patient needs and reports conditions to provider. Receives and returns patient calls within 90 minutes based on medical priority (95% of the time). Accurately records and reports patient’s condition according to department policy (98% of the time). Maintains work areas to ensure healthy environmental conditions. Handles prescription renewals and inquiries, relaying information to pharmacy. Provides appropriate care specific to the age of the patient. Prepares exam rooms and equipment, stocks supplies, weighs patient, checks vital signs, performs medication reconciliation. Assists providers by retrieving tests and records, obtaining lab work. Dispenses medications under provider direction and instructs patients on usage. Assists and/or performs phlebotomy with proper labeling and handling of specimens. Ensures accuracy of order entry and completes appropriate documentation without error (≥98%). Answers phone calls according to protocol (≥98%). Returns phone messages according to protocol (≥98%). Provides test results according to protocol (≥98%). Accurately completes pharmacy refill requests (100%). Education High School diploma or G.E.D. required. Graduation from an accredited Licensed Practical Nurse (LPN) program required. Experience Previous Licensed Practical Nurse (LPN) or medical experience preferred. License/Certifications Licensed Practical Nurse (LPN) license required. American Heart Association or American Red Cross Basic Life Support (BLS) strongly preferred at time of hire; required within 3 months of hire. Skills, Knowledge, and Abilities Knowledge of medical terminology and procedures. Knowledge of medicine including recognition and diagnosis of diseases, deformities, and other illnesses. Effective time management and task prioritization. Proficient computer typing skills. Provides caring patient care services when communicating with patients and families. Critical listening skills to understand patient questions and provider information. Problem‑solving and proactive issue anticipation. Flexibility in daily activities. Proficient navigation of computer programs containing patient information. Strong verbal communication skills with providers, staff, and patients. Ability to follow written and verbal instructions.
