Location: Mantey Heights Rehabilitation and Care Center, Grand Junction, CO

Type: Skilled Nursing Community

Compensation: $40/hr - $55/hr based on experience; Full-time, Part-time, and PRN positions available

Sign-On Bonus: $15,000 for Full-Time roles; $7,500 for Part-Time roles

Role Overview

The Physical Therapist will implement personalized physical therapy programs that encompass assessment, treatment, planning, education, and communication to help patients achieve their functional goals. This role also involves fostering collaborative relationships with other healthcare professionals and support staff.

Qualifications

The following qualifications are essential for this position:

• Knowledge and skills to address the physical and developmental needs of the target age groups receiving services.
• Ability to maintain composure in emergency or crisis situations.
• Graduation from a physical therapy program accredited by the American Physical Therapy Association (APTA).
• A minimum of one year of experience as a physical therapy aide.
• Current licensure from the state Board of Physical Therapy Examiners.

Key Responsibilities

To excel in this role, the individual must effectively perform the following functions:

• Deliver direct patient care, assess functional needs and outcomes, and collaborate with other specialists.
• Review and adjust care plans based on patients’ functional needs to ensure optimal outcomes.
• Develop and implement care plans based on comprehensive assessments, involving significant others when appropriate.
• Conduct therapy practices in accordance with established care standards.
• Guide support staff in achieving patients’ functional goals.
• Communicate effectively with other professionals and support staff to ensure positive patient outcomes.
• Adhere to hospital and departmental policies, quality improvement programs, and safety standards.
• Document evaluations, treatment goals, and plans, ensuring regular updates and maintenance of records.
• Engage in professional development through educational programs, workshops, and in-service meetings.
• Provide leadership and guidance to staff, promoting physical therapy within the community.
• Demonstrate proficiency in reading, writing, and speaking English to meet job requirements.
• Make independent decisions and solve practical problems, interpreting various forms of instructions.
• Exhibit active listening skills and evaluate social situations objectively and fairly.
• Comply with community policies regarding workplace safety, including infection control and emergency protocols.
• Adhere to all organizational policies related to resident rights, abuse prevention, and confidentiality (HIPAA).
• Understand and comply with the Employee Handbook policies.
• Perform additional duties as assigned.
