They tackle non-medical issues and address the root causes: referred to as Social Determinants of Health (SDOH)—like food insecurity, housing instability, and financial challenges that affect patients' ability to heal and adhere to treatment Provide Emotional Support: Deliver vital mental health counseling, offer crisis intervention, and connect patients to behavioral health services. P rovide advocacy & resource expertise: Serve as a strong advocate for patients and families, coordinating various community resources to improve the patient's overall well-being post-discharge. What Minimum Requirements You’ll Need Licensure / Certification / Registration: Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. ACM also accepted. Certified Social Worker preferred. Education Bachelor's degree in Social Work required. Work Experience 3 years of experience preferred.
