Family Practice/Primary Care Nurse Practitioner

The company is a growing, physician-led medical group delivering high-quality care in post-acute and long-term care settings. Founded in 2021 by healthcare leaders with over 30 years of experience, we partner closely with Skilled Nursing and Long-Term Care facilities to elevate care standards, improve outcomes, and reduce hospital readmissions.

Our multispecialty team includes board-certified physicians in Family Medicine and Nephrology, working collaboratively with Advanced Practice Providers to deliver timely, patient-centered care.

Position Overview

We are seeking a dedicated and compassionate Advanced Practice Provider (NP/PA) to provide comprehensive medical care to residents in Skilled Nursing and Long-Term Care facilities, along with structured telemedicine on-call coverage.

This role blends:

Post-acute and chronic disease care
Regulatory documentation and interdisciplinary collaboration
Overnight telemedicine support

The ideal candidate is comfortable practicing independently, managing medically complex geriatric patients, and responding confidently to urgent clinical situations.
