A well-established medical practice is seeking a  full-time Primary Care / Family Medicine Physicians to join a collaborative care team in a community-based outpatient setting. • Evaluate, diagnose, and treat patients using appropriate medical judgment and evidence-based practices
\n
• Provide comprehensive primary care services across the lifespan
\n
• Coordinate patient care, including referrals, follow-up, and care transitions
\n
• Participate in quality improvement initiatives to enhance patient outcomes and cost-effective care
\n
• Maintain accurate and timely clinical documentation that meets regulatory and payer requirements
\n
• Participate in inpatient and outpatient consultation and call coverage as needed
\n
• Engage in academic, educational, or research activities as requested
\n

\n Certifications & Licensure\n

\n
• Board Certified or Board Eligible in Family Medicine or Primary Care specialty
\n
• Active state medical license or eligibility to obtain licensure
\n

\n
• Successful completion of an  ACGME-accredited residency program
\n
