Licensed Practical Nurse-Primary Care

This is a nursing position in the M2Va/TCM clinic. The LPN in this role is a core member of the PACT team. The LPN is responsible for rooming all veterans assigned to the PACT team, completing all required metrics, and reporting to the provider when there are any concerns including positive screenings. Qualifications The GS-7 grade level is for select - complex LPN/LVN positions as established by each healthcare facility. To be advanced, employees must meet all below standards provided they apply to the assignment. Placement of positions at this grade level considers skill mix and availability of supervision in the work unit. These positions are supervised by an RN and/or MD/DO, yet employees demonstrate independent technical judgment and comprehensive knowledge of complex practices and procedures. Individuals may serve as leaders or co‑leaders in a nursing unit or for a specified patient group. Completion of at least one year of qualifying experience at the GS-6 level or equivalent is required. All GS-6 performance requirements must be met. Knowledge and skills are necessary to carry out specialized patient care duties, including assignments in complex inpatient and/or outpatient settings. Knowledge base promotes incorporation of current patient care data to actively contribute to the patient’s overall health plan. Position requires broader knowledge and skills; supervision is general, with staff expected to participate substantially in treatment plan development and modification. Ability to assist the RN and/or MD/DO in prioritizing patient needs and adjusting care plans. Observes subtle physical and emotional changes, documents care delivered, and reports any changes that may necessitate immediate modification of the care plan. Seeks assistance by informing the RN or MD/DO of changes in patient condition needing higher‑level intervention. Participates to a greater degree in prioritizing and modifying patient care treatment at this grade level. Acts on subtle changes appropriately and assists the RN or MD/DO in medication/treatment administration, records responses, and responds to potential drug reactions. Demonstrates broader knowledge and skill than lower‑grade staff. Skillfully recognizes potential urgent or emergent situations, seeks assistance, and intervenes to ensure optimal patient care delivery. Maintains a broader knowledge of the patient treatment process and exercises judgment in necessary interventions. Assists patients and families in adopting healthy lifestyles for better outcomes. Communicates effectively with challenging patients and families, addressing issues such as lack of self‑control or abusive behaviors. Provides creative and initiative‑driven improvements to overall care delivery, including participation in committees, task forces, research projects, or educational materials. Promotes established customer service standards in all aspects of practice. Serves as a resource for other staff on specialized procedures and organized complex patient care. Mentor to other nursing staff in applying advanced technology. Supports RN and/or MD/DO in monitoring unlicensed nursing and non‑medical staff practices. Reports deviations from normal practice patterns promptly. Responsible for monitoring unlicensed nursing and medical staff, beyond lower‑grade responsibilities. Works under general supervision with considerable difficulty and responsibility. Expects independent technical judgment and comprehensive practical nursing knowledge. Seeks educational opportunities to enhance nursing knowledge and leads staff improvement initiatives. Physical requirements: visual acuity, keen hearing, clear speech, manual dexterity. Requires potentially long periods of walking, standing, stooping, sitting, bending, pulling, and pushing. Incumbent must manage work effectively under stress and shift priorities as needed. May be exposed to infected patients and contaminated materials, requiring protective clothing in isolation or invasive procedures. May occasionally be exposed to combative patients due to delirium, dementia, or psychiatric disorders. For more information on this qualification standard – please visit Duties Possesses the knowledge and skills to carry out specialized patient care activities. Uses veteran care data to actively contribute to the patient’s overall health plan. Observes veteran status and implements interventions to improve or maintain optimal function. Understands program regulations and carries out Nursing program objectives following guidelines and requirements. Contributes to developing the care plan for assigned veterans, with updates per policy, and obtains RN or MD co‑signature. Promotes and demonstrates nursing concepts throughout the Medical Center. Collaborates with the Registered Nurse and/or Health Care Provider. Prepares and administers prescribed medications (oral, topical, subcutaneous, intramuscular, intravenous) and performs treatments per established policies and procedures. Observes physical and emotional changes, documents them, and reports deviations to RN/HCP. Takes appropriate actions based on observations and communication, recommending care interventions. Assists other staff in prioritizing veterans’ care needs and adjusting care plans. Employs astute assessment skills, observing both obvious and subtle changes in patient status. Responds promptly to deteriorating conditions in emergent situations, notifying appropriate personnel. Accurately records and reports significant veteran information and analyzes it for necessary modifications. Communicates effectively with challenging patients and families to promote optimal functioning. Uses appropriate treatment modalities and implements practices with complex patients. Works independently without readily available supervision while communicating with the veteran, families, caregivers, professionals, and other staff. Performs treatments, procedures, and reprocessing of reusable medical equipment. Shows creativity and initiative in improving veteran care delivery and patient satisfaction, participating in committees, task forces, research projects, or educational materials. Employs sustainable practices to minimize environmental impacts and supports VA stewardship efforts. Monitors unlicensed nursing or non‑medical staff to ensure safe patient care.
