MDS Coordinator (Registered Nurse/RN)

Position Overview: The RN MDS Coordinator is responsible for coordinating, completing, and submitting accurate interdisciplinary MDS Assessments, CAAs, and Care Plans in compliance with CMS RAI Manual Regulations and all applicable laws and standards. Qualifications Associate’s or bachelor’s degree in nursing from an accredited institution Active RN license in good standing Minimum of two years’ nursing experience, preferably in geriatrics CRN C certification (clinical compliance) CPR certification (must be current) Key Responsibilities Coordinate and assist with accurate and timely MDS assessments, CCAs, and Care Plans Report patient condition changes to the DON Educate care staff on CMS RAI Manual updates and regulations Review and verify assessments and attest to their completion in CMS QIES system Monitor CMS reports for assessment timeliness and develop prevention systems Participate in interdisciplinary meetings and perform staff nurse duties as needed Maintain professionalism, confidentiality, and compliance with policies Provide excellent customer service, demonstrate good judgment, and work effectively within a team Essential Skills: Advanced knowledge of post-acute care regulations, independent decision-making, strong communication, and interpersonal skills, along with physical ability to perform job functions. #J-18808-Ljbffr
