Case Manager

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of programs throughout Arizona. CBI provides residential, outpatient, inpatient, patient‑centered medical homes, medication‑assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness. Case Manager The Case Manager provides case management services in either the community and/or in an office setting to patients and their families. The Case Manager assists the patient and care team in planning, coordinating, and monitoring the patient’s needs and progress. The Case Manager assists patients in gaining access to health services, financial assistance, employment, education, social services, and natural support. The Case Manager assesses patient needs, develops and monitors individualized treatment plans, and ensures services are provided accurately and in a timely manner. The Case Manager embraces the recovery philosophy and promotes a safe recovery environment for clients. Responsibilities Assist patients and their families with planning, coordinating, and monitoring care. Coordinate services to meet individual needs. Advocate for patients to access health services, financial assistance, employment, education, and social services. Develop and monitor individualized treatment plans. Promote recovery philosophy and safe recovery environment. Qualifications High school diploma or GED required; associate’s degree or higher in behavioral health preferred. Minimum six months of recovery from substance use and/or mental health disorders preferred. 1-3 years of experience working with the GMH/SU population. 1-3 years of experience in a behavioral health position providing outreach and engagement activities. 1-3 years of experience in a medical setting preferred. Valid identification required. Current AZ Driver’s License, preferred. 39-month MVR preferred. Behavioral Health Technician (BHT) in accordance with A.A.C.
