Hospice Clinical Manager (RN)

Company: John Paul VNA Hospice - a LiveWell Partners Network Agency

Compensation : Salary, Mileage reimbursed at $0.55/mile, and Payroll processed bi-weekly (26 pay periods).

Shift Type: Full-time, On-site, In office Monday - Friday during normal business hours.

Summary:

As a Hospice Clinical Manager (RN) with LiveWell Partners, you will play a vital role in leading our team of compassionate healthcare professionals in providing exceptional care to patients in their homes. As a top-quartile performer in clinical outcomes and patient experience scores, we are committed to delivering the highest level of care and building strong relationships with our patients and their families. As a clinical leader, you will oversee the day-to-day operations of our hospice services, ensuring that our patients receive holistic care that addresses their physical, emotional, and spiritual needs.

Responsibilities:

• Oversee the coordination of hospice care services, ensuring that patients receive comprehensive care that meets their unique needs.

• Develop and implement effective care plans that promote symptom management, comfort, and quality of life.

• Provide clinical guidance and oversight to a team of interdisciplinary healthcare professionals, including nurses, social workers, and other clinicians.

• Foster strong relationships with patients, families, and healthcare providers to ensure seamless continuity of care.

• Conduct ongoing assessments and evaluations to ensure that patients are receiving high-quality care that meets industry standards.

• Collaborate with the hospice care team to address patient and family concerns, and develop strategies for improving patient satisfaction and outcomes.

• Develop and maintain relationships with community partners, healthcare providers, and payors to ensure access to resources and services for our patients.

• Participate in quality improvement initiatives to promote continuous learning and improvement in hospice care.

Requirements:

• Current RN license in the state of Michigan

• Minimum of 2 years of experience in hospice care

• Previous supervisory experience in hospice or a relatable field
